The design appears visually consistent with Emeralite No. 8734 examples, but the exact model and originality of every component have not been independently verified. Please rely on the photographs of the markings, base, shade and hardware when evaluating the lamp.
The lamp is working. There is a small tear in the cord insulation, which has been covered with electrical tape. Because it is an antique electrical item, it should be inspected by a qualified professional before prolonged use.
