Made in USA by Hazel-Atlas Glass Company in Wheeling, West Virginia
In very good condition with no visible signs of major damage or repair. The rim has a small dip where it appears a small chip once was. It has been filed smooth and poses no immediate danger. There are no cracks, breaks or any other chips.
